Available Immediately
? Direct deposit of paycheck, with up to four separate accounts per employee.

Available After 3 Months Continuous Service
? Your choice of two separate Blue Cross Blue Shield policies are available. Employee portion of costs are provided.
? $10,000 life insurance policy is provided by the company to each employee. Additional life insurance is available at very low rates.
? Dental insurance is available.
? Disability insurance for either accident or illness is provided at no cost. This covers 60% of base income up to $500 per week for up to two years of disability. An additional policy which covers from the second year of disability to age 65 is available at a very minimal cost per week.
? Uniforms are available.
? A tax savings cafeteria plan is available on medical benefits.

? 401K is available at open enrollment periods (April and October). You must be 21 years of age to participate. All contributions are tax deferred savings. You may contribute up to 20% of your annual gross salary.
